Please click on the link below to download and print our New Member Application. These forms must be completed by an authorized school professional and signed by both the Head of School and Board President.

Please enclose a copy of the following documents with this application:

  • Statement of the school's philosophy, mission, and objectives
  • IRS letter of determination indicating 501(c)(3) status
  • School bylaws (complete with any amendments)
  • List of Officers and Board of Directors
  • Most recent statement of financial review or audit
  • Current year budget

Please return this application, the above listed documents, and the application fee of $150.00 to:
